There are two types of initial consultations at this hospital: a typical examination to determine the feasibility of treatment and a consultation to obtain a second opinion (medical consultation).

The physician in charge will review the faxed information, and QST Hospital will fax a reply to the referring medical institution in 1 or 2 days. Please understand that a reply may take longer than 1 or 2 days if there is an intervening weekend/holiday or if the physician in charge is away.
The reply will indicate an appointment on the earliest date possible or a date specified by the physician. If there are dates that would be inconvenient for you, please indicate those on the Patient Registration Form to facilitate the process.

Referrals are made by medical institutions (by fax).

check boxPlease ask the medical institution that is referring you to QST Hospital (the "referring medical institution") to fax the required forms [Patient Referral Document and Patient Registration Form] to QST Hospital.

The referring medical institution must fax the Patient Referral Document to QST Hospital even if you (the patient) have already received this document from the referring medical institution.

check box​Please inquire with the referring medical institution regarding the response from QST Hospital. In addition, please contact the referring medical institution for information such as the appointment date, hospital arrival time, and what to bring to the appointment.

 

What to bring to the initial consultation

We will send a notice to the referring medical institution when the appointment is set.
Please review things to bring on the notice along with the appointment date and time and bring them with you on the day of the appointment.

Information requested at initial consultation

To allow us to follow your condition after treatment at QST Hospital, you will be asked for your permanent legal address. Please confirm your permanent legal address and head of household on the family register before your appointment.

Appointment for second opinion (medical consultation)

Referrals for second opinions are made by the medical institution (by fax).

check box​A second opinion is an opinion provided by a physician other than your primary care physician (family doctor). Therefore, treatment and examinations are generally not allowed at QST Hospital on the day of the second opinion.

check box​Because QST Hospital specializes in radiation therapy, you can receive explanations of radiation therapies such as carbon ion radiotherapy and X-ray therapy at this hospital.

check box​QST Hospital does not provide consultations on areas outside the specialties of the outpatient second opinions provided at this hospital; consultations on topics such as dissatisfaction with your primary physician, medical errors, or ongoing legal disputes; or previously completed treatments.

check box​The fee for a second opinion is as follows (not covered by health insurance):

check box​20,000 yen within 60 minutes

check box​If only one of family members is to be seen that day, please bring the patient's consent form and his/her own photo identification for those coming to the consultation.

check box​A report will be sent to your current primary physician (family doctor). Please note that the documents that you bring, such as the Patient Referral Document, test results and imaging data, generally will not be returned to you.
